Scope
This Website Privacy Policy explains how the public website of Sanity Behavioral Healthcare Systems, LLC (“SBHS,” “we,” “us,” or “our”) handles information. It covers the website only.
Health information created or maintained by SBHS in connection with the healthcare services we provide is protected under federal and Maryland law and is described in our Notice of Privacy Practices. If this policy and the Notice of Privacy Practices ever differ regarding your health information, the Notice of Privacy Practices applies.
Information you submit
The website offers three forms: an appointment request form, a contact form and a general inquiry form. They are intended for general inquiries and appointment-related information, and they collect only practical details so our team can respond to you, such as:
- your name, email address and phone number;
- whether you are a new or existing patient;
- the service you are asking about, your preferred visit type (in-person or telehealth), and a preferred date and time;
- optionally, the name of your insurance plan (never your member ID number); and
- a short general message or question.
For your protection, please do not submit any of the following through website forms or ordinary email:
- detailed medical histories;
- psychiatric records;
- medication lists;
- Social Security numbers;
- insurance member identification numbers;
- medical records;
- detailed symptoms; or
- other sensitive health information.
Clinical details can be discussed privately with your provider during your appointment.
How form submissions are delivered
When you submit a form, your web browser opens your own email app with a pre-written message addressed to SBHS at ajetasbhsystems@gmail.com. Nothing is sent until you review the message and press Send in your email app. If SBHS later connects the forms to an online form service, submissions will be sent directly to SBHS and this policy will be updated.
Website forms and ordinary email are not a secure clinical communication platform, and they are not emergency communication channels. Information submitted through the website may be routed to SBHS for follow-up according to SBHS’s applicable privacy and security practices.
How we use website information
We use information submitted through the website only to:
- respond to your message or question;
- contact you about scheduling and your appointment request; and
- carry out related administrative activities of our practice.
If you become an SBHS patient, health information connected with your care will be handled as described in our Notice of Privacy Practices.
Sharing of information
SBHS does not sell information submitted through this website and does not use it for advertising. We may share it only as needed to respond to you and operate our practice, or when required or permitted by law. Protected health information is never sold except with a legally valid authorization where required by law, as described in our Notice of Privacy Practices.
How long we keep information
We keep information submitted through the website only as long as reasonably needed to respond to you and to operate our practice, or longer where required by law. If you become a patient, your records are kept as required by applicable federal and Maryland law.
What the website stores on your device
This website stores only one item in your browser: your light/dark display preference (sbhs-theme). The website itself does not set cookies and does not store form entries, names, contact details or health information in your browser. You can clear this setting at any time through your browser settings. The embedded Google Map on our Contact page is provided by Google and may use its own cookies under Google’s policies.
Analytics and tracking
This website does not currently use analytics, advertising or tracking tools. If SBHS adds such tools in the future, this policy will be updated first.
Third-party services
To display the website, your browser loads fonts, icons, styling tools, images and an embedded map from third-party providers: Google Fonts, Font Awesome (via cdnjs), the Tailwind CSS CDN, Pexels and Google Maps. Like any website, these providers may receive standard technical information such as your IP address and browser type. Their own privacy policies apply to that information.
Your choices and rights
- You may contact us to ask what information you submitted through the website, or to ask us to correct it.
- You may ask us to contact you at a particular telephone number, mailing address or by another reasonable method.
- For your rights regarding your health information (such as access, correction, restrictions and an accounting of disclosures), see Section 1 of our Notice of Privacy Practices.
Children and adolescents
SBHS provides care to adolescents and adults. This website is not intended for children under 13 to submit information themselves. A parent or guardian may submit a request on behalf of a minor.
Privacy rights concerning minors can depend on federal law, Maryland law, the nature of the healthcare service, the minor’s legal status and parental or guardian authority. See Section 8 of our Notice of Privacy Practices.
Security
We take reasonable steps to protect information we receive. However, no website or ordinary email transmission is completely secure. That is why we ask you not to send sensitive health information through the website.
Complaints
If you believe your privacy rights have been violated, you may contact the SBHS Privacy Officer using the details below. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at https://www.hhs.gov/hipaa/filing-a-complaint/ (opens in a new tab). SBHS will not retaliate against you for filing a privacy complaint.
Changes to this policy
We may update this policy from time to time. The “Last updated” date above shows when it was last revised, and the current version will always be posted on this page.
