Not for emergencies
SBHS is not an emergency or 24-hour crisis service. Website forms, email, and appointment requests are not continuously monitored. For an immediate medical or behavioral emergency, contact emergency services (call 911) or seek the nearest emergency department. You can also call or text 988 to reach the 988 Suicide & Crisis Lifeline.
Appointment requests
Submitting a form does not confirm an appointment. Appointments are confirmed only when SBHS contacts you directly. Availability of services, including telehealth, depends on the service, provider, patient location, licensure and applicable requirements.
Using website forms
Website forms are intended for general inquiries and appointment-related information. You agree to provide accurate contact information and not to submit any of the following through website forms or ordinary email:
- detailed medical histories;
- psychiatric records;
- medication lists;
- Social Security numbers;
- insurance member identification numbers;
- medical records;
- detailed symptoms; or
- other sensitive health information.
Website forms and ordinary email are not a secure clinical communication platform.
Privacy
Our Website Privacy Policy explains how this website handles information. Our Notice of Privacy Practices explains how we may use and disclose your protected health information and your rights regarding it.
No online payments or accounts
This website does not process payments and does not offer user accounts, logins or a patient portal. Please contact the office with any questions about insurance or cost.
